Please answer each question to the best of your ability. All answer types are smart phone enabled, ranging from texts, dates, photos, and more.

MAKE SURE TO ANSWER EVERY QUESTION

Most of the answer fields are required, so it’s best to answer all of them. Otherwise, you’ll run into issues when hitting the “submit” button.

SUBMIT YOUR FORMS SO WE KNOW TO GET YOU ON OUR SCHEDULE

Once you’ve filled out all the answer fields, you’re ready to hit submit! Doing so will notify our scheduling team to give you a call.

2. Scheduling

KEEP AN EYE OUT FOR A CALL FROM OUR SCHEDULING TEAM!

Our scheduling team is notified as soon as you hit “submit” on your mobile new patient forms. They’ll reach out to you up to 3 times to get you on our appointment schedule, so please make sure to answer!

Once they get ahold of you and get you on our schedule, they’ll send you an appointment confirmation text. Make sure to mark the date and time in your calendar, and we’ll see you soon!

3. Virtual Appointment

Similar to the text & link to your new patient forms, clicking this link will open up your smartphone’s internet browser to a smartphone browser-friendly video chat website.

ALLOW ACCESS TO YOUR MICROPHONE AND CAMERA

Your smartphone will immediately prompt you if you’d like to give access to your microphone and camera. Please hit “allow”, because this will let our provider see and hear you!

CLICK THE GREEN “JOIN” BUTTON TO INITIATE THE VIDEO CHAT

You can immediately join the video chat by clicking the green “Join” button. Otherwise, it will automatically start in a few seconds. There are also buttons you can toggle for muting your mic or disabling your camera.
